Part B includes TWO sections: 
In the first section, you will sign a release for your provider to provide us with the needed information to properly train your service animal.
The second section is the part your provider will complete.

You can download the form using the button below, print it and take it to your provider, or email it to their office.  Once completed, they can either return to this page and hit the button that says, "Email your completed form," they can mail it to us at the address below or it can be scanned and emailed to us at:  admin@pawsforlifeusa.org.
